Patient segmentation categorizes patient populations into homogeneous groups based on their level of risk (high-risk, at-risk, and normal) and disease state. To perform effective patient segmentation, IDSs must develop new screening models and criteria, generate or locate patient population data sufficient to allow effective screening and segmentation, and create new incentive structures to encourage early screening and diagnosis. The considerable investment of time and effort in patient segmentation promises a large payoff by helping IDSs manage their highest-risk populations effectively."
