The solution is to set date.timezone in php.ini, if the TZ environment variable isn't set:

Phil Taylor Releases New Forms Component

Having a form on your website is incredibly useful. Its the easiestway to get information from your site visitors, which is critical foran interactive site. For such a useful and obvious extension, itsalways surprised me that there were not many options, and even fewerthat work on 1.5.
