Sam is compensating for having Lucifer in his head by going into hyperfunctional mode. I want to see touches of Mystery Spot OCD Sam: hospital corners, perfect efficiency, everything under control, everything in place. It works. He still hallucinates, but he can keep going through it, he can wall himself off from being affected by fear of his breaks with reality and by the trauma of the things he remembers and hallucinates. He just has to keep going further and further into his coping mechanism, because if it breaks it will all come crashing down and that will not be good.
The breaking of the Wall let out a lot of things better left alone, memories not the greatest of them. Set after S01E06 Skins. The first night it happened, Dean tensed up, ready to wake Sam from whatever nightmare he was fighting. Except after a second, he realized Sam's moans didn't exactly sound like he was scared or distressed. Or not distressed from fear or dread.

When they manage to get Sam's soul back into his body, he isn't so sure anymore. Instead of a cold, unfeeling shell he gets an insane wreck who is completely overwhelmed by the memories of centuries of neverending physical and emotional torture. Cas tried to warn Dean that this would happen, but Dean thought he could deal - and anything's better than leaving Sam's soul in hell forever.
Unfortunately, Dean can't deal with this on his own and eventually Sam ends up in a mental hospital, drugged and restrained to keep him from hurting himself and others in his madness. Unable to tell reality from his memories to begin with, the restrains make Sam freak out even more. Completely refusing food, plagued by horrific nightmares whenever they drug him to sleep and under constant emotional stress, he wastes away and falls sick. Castiel sticks around to help him as best he can, because he somehow feels responsible for Sam, or just because Sam is his friend. Dean can't bear to see his brother suffer like that but stays anyway because he's an awesome big brother.

My favourite Supernatural stories! Mostly featuring !hurtSam & other stuff. Wincest kinda icks me out but I'm fine when they're unrelated. Still loving the slash!
