Clinical commissioning: our vision for practice-based commissioning : Department of Health - Managing your organisation

  • The Department of Health has worked with a wide range of clinicians, PCTs and strategic health authorities to produce a collective vision for clinical commissioning. This vision is presented in the document ‘Clinical Commissioning: our vision for practice-based commissioning’, which was launched on 4 March 2009.
    The document sets out the hallmarks of successful clinical commissioning, the support and entitlements that practice-based commissioners can expect, and the principles underpinning vibrant, productive practice-based commissioning partnerships between PCTs and their clinical communities.
    It confirms the direction of practice-based commissioning and firmly embeds it as a core aspect of World Class Commissioning.

Practice-based commissioning budget guidance for 2009/10 : Department of Health - Publications

  • PCTs are responsible for ensuring that practices receive an indicative budget that reflects the needs of their population as accurately as possible. This allows a practice to access a 'fair share' of the resources available to the whole of the PCT for its patients. The Department has updated the toolkit that can be used to determine weighted capitation indicative budgets at practice level. This is part of our ongoing commitment to refine the methodology and improve accuracy.

Guidance on the routine collection of Patient Reported Outcome Measures (PROMs) : Department of Health - Publications

  • This document contains detailed guidance on the routine collection of Patient Reported Outcome Measures (PROMs) for elective procedures from 1 April 2009. The document is intended to support providers and PCT commissioners to implement the requirement to collect PROMs contained in the standard NHS contract for acute services.

Transforming community services: currency and pricing options for community services : Department of Health - Publications

  • This document is intended as a resource to be used by commissioners and providers of community services to help create transparent models of funding, through the use of new currencies and better pricing at a local level. New currencies (the units of healthcare for which a provider is funded) and better pricing are key to transforming community services, enabling commissioners to incentivise improvements in quality and value. The use of new currencies with accurate pricing should also lead to more services being available closer to home for patients.

Cancer commissioning guidance : Department of Health - Publications

  • The cancer commissioning guidance announced as part of the Cancer Reform Strategy has been developed to support world-class commissioning of cancer services across the NHS. The guidance sets out, in an easy-to-use format, key issues and questions that commissioners and cancer network teams will want to take into consideration when assessing health needs, reviewing services, developing their contract service specifications and monitoring performance.
    The cancer commissioning guidance sits alongside the linked web-based cancer commissioning toolkit which supports commissioners of cancer services by providing a range of benchmarked information and data.

Transforming community services and world class commissioning: resource pack for commissioners of community services : Department of Health - Publications

  • This resource pack sets out the principles of good practice when commissioning community services, showcases examples of existing good practice through case studies in 6 clinical areas and signposts commissioners to the most pertinent and relevant information.

Learning disabilities : Department of Health - Social care

  • Valuing People Now: Transfer of the responsibility for the commissioning of social care for adults with a learning disability from the NHS to local government and transfer of the appropriate funding
    In August 2008, a letter and annex was sent to all primary care trusts and local authorities setting out the existing policy and legal framework for the transfer of learning disability social care funding and commissioning from the NHS to local authorities with effect from 1 April 2009. The annex provides guidance on what is covered, what PCTs and LAs need to do and the timescale for the transfer. The proposed transfer was set out in Valuing People Now.
    A further letter in November 2008 gave an update and specifically advised PCTs and local councils as to what the DH were looking for from the December 1st return.

Care planning : Department of Health - Health care

  • This guide will provide commissioners of health and social care services with the information and support they need to embed personalised care planning in their localities. This should ensure that people with long term conditions receive more individualised care and services to help them manage their conditions better and achieve the outcomes they want for themselves. The guide has been developed in collaboration with stakeholders across health, social care, the voluntary and independent sectors and patient representative groups.

Common Assessment Framework for Adults: A consultation on proposals to improve information sharing around multi-disciplinary assessment and care planning : Department of Health - Consultations

  • The Department has launched a consultation on improving the quality and efficiency of care and support through improvements in the sharing and use of information. The consultation focuses on care and support for adults and on assisting the continued development of personalisation by helping people to choose services better suited to their needs. The document covers the rationale for improving information, how assessment and care planning should be undertaken (the principles), what information should be commonly shared and with whom and the IT approaches that would enable this to take place within appropriate arrangements for consent, confidentiality and security. The consultation will be of interest to the general public, commissioners of health, social care and wider community support services, the voluntary and independent sectors.

  • Further to the publication in March 2007 of Improving access to audiology services in England, this document provides good practice and evidence to help commissioners and service providers to make changes to the way that adult balance services are delivered, and in particular to reduce waits for patients with the most common hearing difficulties.

  • Lord Darzi announced in High Quality Care for All that the Department will launch a pilot of personal health budgets in 2009, as a way of giving people greater control over the services they use and who provides them. Personal Health Budgets: First Steps sets out the principles of personal health budgets, reports on early lessons and invites expressions of interest in the pilot programme.

  • Vascular diseases include heart disease, strokes, diabetes and kidney disease. A univeral risk assessment and management programme could significantly increase uptake of the preventative interventions, and offers a real opportunity to reduce health inequalities.

Understanding detailed patient experience data: toolkit for analysts : Department of Health - Publications

  • The Department of Health has developed a Patient Experience First Steps Diagnostic Tool to aid understanding of data from the national patient survey programme. The overall aim of the tool is to help organisations identify areas where they can improve the experience of patients. This in turn will help improve their patient experience PSA scores (PSA delivery agreement 19, Indicator 1: Ensure Better care for all) and “Vital signs” scores (VSB15 – Self-reported experience of patients and users).
