To me, these constraints scream "elevation-aware perfect maze generation algorithm", rather than "fractal". A simple maze generation algorithm that repeatedly places a link at the cheapest transit it can find to an linked zone should work, and should generate believable rivers that fit the above conditions.

Have different types of transit. Unless deliberately isolated, they still would have methods of communication / trade. It just may be that they can travel more efficiently on a river.
