The mitral component of the first heart sound is usually soft and is followed by a pansystolic murmur which is high pitched and extends, as the name suggests, the whole of systole. The murmur usually radiates to the axilla. The loundness of murmur does not correlate well with the severity of regurgitation. Patients may also have a third heart sound.
