    I do informal and formal conferneces - the students really like the feedback and i ask them to write a reflection on what they learned during the conference. It also gives them a chance to tell me something they weren't comfortable saying or forgot during the sit-down converation.

    I have been looking for a checklist like this!

  then instructs students to form a few new self-direction goals based on their
  answers to the checklist.

    I like the idea of building on self-direction goals and am trying to figure out how to keep up with those goals. I use a paper folder for each unit - maybe I could have students put the goal in a designated spot. each time we get a new unit we see the other packets anyway.
