It is required that any school, group, or individual teacher who wishes to use ProfilerPRO with students 13-18 obtain and hold written parental permission prior to the registration of these students. This parental permission form should include the following information:
* consent to have the student registered with an on-line collaboration tool in which the student's name, e-mail address and survey responses may be shared with his/her peer group;
* a copy of the entire survey and all accompanying instructional materials to be used in conjunction with that survey;
* how the data generated by the survey will be used;
* how the parent can request the removal of the student's data.
